Assisting with Medication

The Certified Nursing Assistant (CNA) or Home Health Aide (HHA) may need to assist a client with the administration of their medications. This is a task that carries huge responsibilities and requires specific training for those individuals who will be assisting a client in these activities. This training is designed to promote a clear picture of the role of unlicensed personnel who will be assisting a client with their medications. It is important to note that in the state of Florida, to assist is not to give the medication, but rather to simply help the client to take their medications. These medications include those that are prescribed by a physician as well as over the counter (OTC), or non-prescribed medications. This assistance with medications may occur in a home health setting or an Assisted Living Facility in which the client may be residing. Florida Chapter Administration Code 58A-5.091 states the healthcare provider must complete an initial 4 hour training session followed by an additional 2 hours of training annually on providing assistance with self-administered medications.

Communicating with the Congnitively Impaired

Health care workers frequently encounter individuals who are cognitively impaired, Cognitive impairments affect the ability to think, concentrate, formulate ideas, reason and retain information. Although often associated primarily with old age, cognitive decline may occur due to a variety of reasons such as:

  • Alzheimer’ disease
  • CVA (stroke)
  • Delirium
  • Brain trauma
  • Developmental disorders
  • Dementia

The signs of cognitive impairment include memory loss, changes in behavior or personality, loss of judgment or the ability to make decisions and other communication difficulties. When a person suffers from cognitive impairment it can be challenging to communicate effectively with them. As healthcare workers, it is very important that we are able to have the patience, understanding and skills to provide quality care to those individuals who are affected by cognitive impairment.

End of Life Care

There is no ‘one size fits all’ when it comes to death. The only guarantee is that it will happen to all of us. Yet Americans remain strangely reluctant to discuss the concept of their own end-of-life preferences. In 2010 the National Hospice and Palliative Care Organization (NHPCO) stated that less than one third of Americans have placed their wishes in writing and the other two thirds have not thought about the issue at all, or have not shared their thoughts with others. This unfortunate avoidance of topic is compounded by a medical community that often feels uncomfortable discussing a terminal diagnosis with a patient and may associate death with the “failure of treatment.”

Domestic & Family Violence

Domestic & family violence is the most reported violent crime in America. The family unit is a complicated system and abuse in any part may lead to issues that affect the family as a whole. Every person, young and old deserves to be treated with respect and to be safe from harm. Unfortunately, family violence (the abuse of a child, intimate partner or elder) occurs throughout our country every day. The focus needs to shift from any acceptance of family violence to one that empowers families and communities through education and advocacy programs that support a zero tolerance for abuse of any form. This can be achieved by a change in family and cultural attitudes, which carry a strong message that violence is never acceptable and often has lifelong negative consequences.

HIV / AIDS and Infection Control

All healthcare workers have an obligation to provide care in a manner that minimizes the risk of infection to the patients they provide care for while also protecting themselves from infectious agents. Healthcare facilities have policies in place that must be followed to ensure compliance. These policies include environmental control measures as well as safe practices regarding specimen collection and exposures to pathogens. These standards are set forth by various government agencies that have been tasked with providing guidelines while monitoring their effectiveness.

Professional Documentation - HIPAA and Patient Rights

The health record is an important source of data of the care provided to the patient in the healthcare setting. This record also becomes a legal document that can be released upon request to various parties. In addition to the legal aspects of proper documentation, this record validates adherence to standards and becomes a resource to the clinical team for care planning and education purposes. Accuracy of medical record documentation is also imperative to the insurance industry to ensure that payment will be received for services that are rendered to a client. Privacy of the client’s information must be strictly enforced under the guidelines of HIPAA policy. The healthcare system in the United States is constantly changing and evolving and it is a responsibility of all members of the healthcare team to focus on accuracy and privacy of patient medical records while ensuring the protection of their rights.

OSHA Bloodborne Pathogens

In 1991, OSHA (occupation Safety and Health Administration) issued in the Bloodborne pathogen standards; which must be followed in all workplaces where employees can be expected to come in contact with blood or other body fluids. The standard’s requirement states what employers must do to protect workers who are occupationally exposed to blood or other potentially infectious materials (OPIM). It was revised in 2001 and included Needle Stick Safety and Prevention Act. Bloodborne pathogens are listed under Federal Law 29 CFR 1910.1030.

Skin Infections

As health care workers, it is important to be aware of skin infections and the proper precautions to be taken when encountering individuals who have them. The skin provides a good barrier, which helps to prevent infections. Although many bacteria may come in contact with or reside on the skin daily, they are normally unable to establish an infection. When skin infections are present, they can range in size from a tiny spot to the entire body surface. They can range in severity as well, from harmless to life threatening. It is an important responsibility of the healthcare provider to be able to recognize a variety of skin infections as well as to understand the treatment dynamics associated with various infections and skin conditions.

Wound Care

The assessment and treatment of wounds has become a specialty field in healthcare in the past several years. It is important for all healthcare professionals who have contact with patients to understand some basic concepts for wound care to assist in the proper diagnosis and treatment of wounds. As our patient population ages we find more opportunities to impact the care delivery system in the field of wound management. The economic impact of wounds in the health care system is startling, estimated to be approximately 6 billion a year in pressure sores alone.
